My name is Forhad Hossain. I am a PhD student at the faculty of information science and electrical engineering at Kyushu University. My PhD research goal is to add value to doctors’ decision making processes by visualizing a patient’s lifelong medical history. Towards this goal, I have developed a Smart Health Gantt Chart system. In order to evaluate the performance of the system, I am collecting user observations. Your observations will help us design a better system.

We are aware of the sensitivity of the personal information. We will try our best to maintain your privacy with the available technology we have at our disposal. We are outlining our responsibilities below.

Acceptance of Terms:

1. SHGC will collect your personal profile, medical history, treatment plans, and health progress information. You do not need to provide all the information. However, more information will create a better gantt chart. Please provide as much as you can.
2. We ensure to maintain the security and confidentiality of your personal information. All data collected will be stored securely and protected against unauthorised access or disclosure.
3. We will not share any private information such as your name, address etc.. However, we will use secondary statistics for our research analysis and publications.
4. We request you NOT to provide any false information. A false information will lead to a false output.
